Genuine uchihas unlock them when they experience emotional trauma, causing their brains to release a special chakra that reacts with their optic nerves to produce sharingans, according to tobirama.

If onion is an genuine uchiha, he can release that chakra, but I'm not sure if it will react with sarada's eyes.

If he is a fake, he cannot release that chakra and no sharingans can be produced.

Then again, we've seen how kakashi unlocked his MS after witnessing Rin's death. if it's because of the visual link between him and obito (Obito unlocked MS by the mentioned mechanism, kakashi got it too due to the chakra linkage), then the same can be applied to onion and sarada, and that would depend on whether sarada has suffered enough to unlock 3 tomoes
