Depends. Is itachi suffering from the disease or is that negated? Plus how much time has he had to master the ems techs. Depending on that, itachi might or might not have perfect susanoo, which would stand up to any rinnegan attacks, and considering itachi has yata and totsuka, I'd give this to itachi.plus he's never shown us his true powers, even kishi said so.
I wouldn't count on meteorite working, cause he can just use izanagi for his eye. Plus even without ems and perfect susanoo, it's a question of whether or not totsuka works on perfect susanoo. It's defense is perfect but the blade is ot material but ethereal, so there's no reason it shouldn't work. I'd give this to itachi with very high diff, since it's Edo Madara, meaning he doesnt die, which includes, itachi might sacrifice his eye in the fight.
And for people hating, face it. Itachi WAS the strongest uchiha that ever lived, in potential. By feats, he was above any other uchiha in history, and just like minato, he had the potential to become the greatest ninja ever to walk the earth, if his life hadn't been cut short.

are we talking edo itachi, sick itachi, or healthy itachi it makes a big diff edo then yes madara would probley lose thanks to sword of totsuka(idk how to spell) and the yata mirror can deflect anything madara throws even his meteorite technique against sick itachi then edo madara without a doubt thanks to his inability to die his chakra reserves and stamina healthy itachi would put up a hell of a fight but would need to end it quickly because he would run out of chakra so to sum it up imo

edo itachi> edo madara med diff
sick itachi<edo madara low diff
healthy itachi> edo madara extremely high diff

Madara was a legend Back in the days.He is the true master of the sharingan .He knows everything about the sharingan
also his fire techniques are stronger,he has more stamina,better taijutsu and better genjutsu (moon eye plan) .He fought Hashirama many times before the valley of the and they were almost equal (hashirama still litle stronger)
So no itachi is weaker than ems madara
