Assuming he didn't die- Tsunade wouldn't have become Hokage-
Thus not teaching Sakura anything- thus no support on getting Sasuke back-

Pain would have been neutralized, Orochimaru would die in vain, Kabuto would cooperate with Konoha, Sasuke would never leave the village, the ten tails would never have been released, and black zetsu would ultimately be neutralized. Peace would be achieved immediately.

If hiruzen didnt die Then all this would happen:
-sasuke would be dead long ago because he couldnt let sasuke roam free in his era. All sauce fans would have suicide long ago.
-Akatasuki members would be buried altogether alone by hiruzen except itachi.
-Madara would be defeated and then sealed alone by 3rd hokage
- Hagoromo wouldnt show his self because he knows that someone stronger than him living on earth
In Short, no IT and no hagoromo bullshi.t happened if hiruzen survived.

well orochimaru would be dead so sasuke wouldn't have a good teacher and would remain fodder while naruto still skyrockets to the top thanks to jiraiya. tsunade doesn't come to the village so when pain attacks an old as f*ck hiruzen gets curb stomped and the village gets raped with large amounts of casualties including sasuke who probably tried to back up naruto but only failed and this would throw naruto over the deep end and he'd go full berserk with no chance of coming back and pain captures him and extracts the beast. but even implying naruto still TNJ pain it only makes things worse because now we've reached the war and theres no orochimaru to bring back the hokages so madara and obito obliterate the SA.
^^with all that happening the last few chapters of the manga are madara vs jj obito and madara wins and then casts IT

I don't think he would've done much, he was old and would probably get negged by Pain.
He would have used special Sarutobi medical ninjutsu for Sasuke and Kakashi's recovery. Thus making them full powerhouses, Sasuke wouldn't have gone to Orochimaru and Kakashi would have got his 2nd MS by the end of part 2.
Also:
*Tsunade would end her days gambling, or sweeping Hiruzen's floor
*Naruto < Sasuke
*Kaguya would have died by the simple fact that Hiruzen splitted his chakra
*Hidan would have become mortal
*Naruto would have had his Bijuu removed w/o dying, Kurama becoming The Professor's pet
*Jiraya would have seen Naruto becoming a mere servant of the Double Rinnegan Master, Sarutobi Sensei
*The Otsutsukis would have been referred as the wannabe Sarutobis
*Hiruzen would have made Sakura relevant to the story(although a bit of a short story)
*Lord Third would have used the 9 gates to destroy Kamui, just for fun
*The Professor would have found Tobidara using Creation Of All Things
