Its hard to control and the body part that is inplanted with it can repid grow into a tree and tear you appart. Danzo lost control it for a short time but could stop it in his fight with Sasuke.

You need to be a good match up for the DNA otherwise it malfests and grows into its own organism usually killing the host. In Oro's experiments most victims died, Yamato was the only true success (Uchiha are apparently close enough relatives that it syncs up well). Danzo wasn't a very good match up and when he lost control over it it grew out of his body and became a tree.

Depending on the recipient, it can range from death (Orochimaru's experiments) to uncontrolled growth of implanted cells (Danzo's arm) to inability to poop (Obito). Of course, in the rare case, the side effects could also be... nothing (Yamato).
