Madara didn't go all out (though it wouldn't change much), but that Gai almost killed him is stated by Madara.

He did not go all out. He was kind of serious because he expressed concern if Gai kept hitting him and he protected himself when Gai was going for the last Elephant hit. I believe he was serious but not all out.

Plot restricted Madara into standing still.

but what if madara just wanted a formidable opponent that could cause him pain and that's the reason he didn't fight back?

I don't see why not? Madara didn't fight properly, but he tried to defend against his attack, but failed. Guy soloed him and Madara even stated himself that he almost got killed. For me, I think Madara could not perform any strong moves to avoid or counter Guy's technique, and in other words, Madara did test him, however as it seems he wasn't able to use any of his jutsus because Guy was too fast for him. There you go.

Madara said the situation was precarious but he still had fun. Why was it precarious you say? Because he could do nothing to defend himself

He was playing. He could have finished him off. 8 gates did damage tho. I guess he was playing but not expecting such a force.

Madara was toying with him.he took most of the hits because he could. He liked the fact that gai was actually able to hurt him. Gai caught him off guard with the night moth but thats the only thing he really got on madara. I know most people refuse to accept it but madara hasnt really so much as warmed up yet

I mean, I hate to take a dump on peoples parade here but madara hasnt had a reason to be serious until now. No one hes faced so far has been strong enough to make him even worry. As much as I hate to say it myself, if he gets his other rinnegan, sasuke and naruto are going to be in for a hell of a fight, even with thier new strength.
