Defensive Kamui, means teleporting something away that will hit you, to protect yourself. Kakashi never had intagibility, before he got his second MS(From Obito).

That's subjective. Kamui itself is neither defensive or offensive. OP needs to specify.

This is kakashi who can kamui other objects and himself since madara was able to kamui himself with kakashis eye. i assume that the latter was able to do it too.
Healthy itachi is the same itachi with no spying out blood, his chakra levels are still the same.
