If he was recognized, old man kage (Hiruzen) could have avoided death at Oro's hands.
So why do you guys think that no one, Hiruzen, leaf elders, Anbu leaders recognized him.

And could Danzo have placed him with Oro on purpose to overthrow the 3rd. So that he could become hokage?

It would seem the hokage has files on every shinobi Anbu and Root included. The root only take orders from Danzo though.
And remember Danzo had given Orochimaru a file containing information about the Hidden Leaf's anbu including names.
The 4th raikage also suspected he played a part in Hiruzen's death.

He was part of ANBU root, I don't think regular ANBU members or Hiruzen even knew who they were. Also Kabuto was away from the village for the most part doing his whole spy thing so it's likely he spent very little to no time in the village. Even if he was in the village he would probably be wearing the ANBU mask at the time, or be placed in a secluded place to minimize any chance of him coming into contact with 'mother'

Because Danzo's ANBU Root's members are all covert operatives meaning they operate in secret, and even have aliases rather than names. No one even knew that Danzo's minions were apart of the Yamanaka clan and Aburame clan because they were always behind masks.
