i have been wondering
when an Uchiha has to take the eyes of their brothers for EMS do they actually take out their eye and put the eyes of the brother in the empty place were the eye are ment to go?? and if so why don't they just swop eyes and they will both get EMS (that is if both of them have MS)
what do you guys think about this?

This is a very good thought, I believe that two uchiha's would gain from this process. I dont see why tyhey would not.

According to wikia, Izuna managed to live but died in the battlefield due to his disability.

Well, swapping each other's MS may be plausible, though it wasn't dictated in the circumstances. First, Madara has gone blind before he got his brother's eyes that's why there's no use for swapping. The same thing goes on Itachi being already dead, and Sasuke being almost blind before he took Itachi's eyes (supposedly) for the EMS.

If I recall, Madara was the first Uchiha to gain EMS. It may be possible that no one knew yet about the EMS (more so about taking your brother's MS eyes in order to gain it). I'm thinking that Madara mainly took Izuna's eyes coz he's gone blind and he's struggling to gain his MS techs back again and be powerful. I'm assuming that the EMS was a "blessing in disguise". Hence, that's how getting an EMS is revealed.
