I've played with a couple options for a timeline for Obito/Madara/Tobi that attempts to put certain events in order, and attempts to explain why we've seen masked men with different masks and different lengths of hair. This is the only sequence I've found that mostly works. It's not perfect, but it's the best without producing any major plot holes. First I've included a raw timeline and after that I've written up some questions and answers about some of the details.

Unified Tobi/Madara/Obito Timeline:
1) Madara pretends to die in battle against Hashirama
2) Madara convinces Yahiko to create Akatsuki, and gives Rinnegan to Nagato.
3) Obito "dies" in battle, but his MS activates at the last minute and he's able to phase through the rock to safety.
4) Obito witnesses the death of Rin. He blames Minato/Kakashi for her death and decides not to go back to Konoha
5) Obito meets Madara, learns of his plan, and decides that he wants to destroy the whole world to avenge the death of Rin.
6) Madara realizes that he is going to die soon and starts wearing a mask. Obito starts wearing the same mask and plans to grow his hair long like Madara's.
7) Obito, with a mask on, goes to Konoha to get revenge against Minato: kill his wife. Kill his son. Release the fox to destroy the village.
8) Obito travels to Kirigakure to become the Mizukage. This way he can hide in secret while growing his hair long. Pain is not aware that the Mizukage is being controlled by Obito.
9) Madara, with mask on, joins Itachi in the Uchiha massacre
10) Some time later, Madara dies
11) Obito, with the same mask and long hair, starts calling himself Madara Uchiha. Pain doesn't notice the difference and continues to take orders from the masked man
12) Obito decides to join Akatsuki since the members keep dieing. He cuts his hair short again and changes to the new orange swirl mask (because Kisame might recognize his siloette with the mask on) and calls himself "Tobi"
13) After Pain and Deidara die, Tobi reveals himself to be "Madara". Kisame recognizes his face (Obito's face) as the Mizukage.

Questions and Answers:
Question: Madara said he unlocked the Rinnegan shortly before his death, but this timeline has him surviving for at least another decade, during which time he must have had eyes to help massacre the Uchiha
Answer: Yes, this is the one big flaw with this timeline. Either Madara lied (or exaggerated) about the amount of time between unlocking the Rinnegan, or else he had a backup set of eyes to use for a few years after giving the Rinnegan to Nagato. If the Madara Itachi saw during the massacre was the real Madara, and if Itachi said he was a "shell of his former self", that suggests he might have been using a weaker set of eyes if it was indeed the real Madara and not Obito. Also, if Madara knows Obito, he had to have survived until at least Obito's apparent death in the third world war (after having given Nagato the Rinnegan in the second world war) so one way or another it was a longer time than we thought.
Question: How did Madara live so long, if he indeed died during or after the third world war?
Answer: Hiruzen and Danzo were both adults during the first world war. Assume Madara was about 20 years old when he died, and he died a few years before that. So he wouldn't have been too much older than Hiruzen (and again, his age is one reason why Itachi might have called him a "shell of his former self"). Also, Tsunade mentioned that the addition of Hashirama's DNA would have made him immortal, which we already know didn't completely happen because he did die. So at least it gave him a little bit more longevity.
Question: Why did Madara help massacre the Uchiha?
Answer: Revenge for Izuna. Izuna gave his eyes and his life to protect the clan. The clan abandoned Madara and said he took the eyes by force. This means that Izuna's sacrifice was wasted and forgotten. Also, it was an interesting way for him to test his abilities, after having avoided big battles for many years, and having given away his most valuable set of eyes to Nagato (but having gained some of Hashirama's powers too). Also, if you're creating a world-wide genjutsu dream, you want to kill people who you think are not worthy before making them a part of your permanent dream, or killing people whose eyes might help them break free from the genjutsu. The Uchiha clan's sharingan is one of the only natural defenses against sharingan-based genjutsu, which means every member of the clan was a threat to the plan.
Question: Why did Obito invade Konoha, fight Minato, and try to kill baby Naruto?
Answer: Revenge for Rin. Obito clearly blames Minato for Rin's death more than he blames Kakashi. That's why he didn't outright blame Kakashi in chapter 600. This is also why Obito chased Minato around the village and insisted on fighting him, instead of capturing the fox and taking the fox away to Akatsuki headquarters.
Question: Why did Obito and Madara both wear masks?
Answer: To set up a succession for when Madara died. Madara was not immortal, even if his life was longer than normal. In order to pretend to be immortal (and keep the respect of Pain and Konan), he had to pretend that Obito was Madara after the real Madara died. You do this by having both of them wear masks and growing their hair to the same length. When Madara dies, Obito just pretends to be Madara like nothing happened. Since Pain had different plans about the goals of Akatsuki (using the Bijuu to make a weapon), they couldn't turn control of the organization over to him. Only "Madara Uchiha" would use the Bijuu for the correct purpose.
Question: Why did Madara's mask only have one eye hole, when Madara had two eyes?
Answer: Because Obito's mask only had one eye hole, because originally Obito only had one eye. They needed to be the same mask, and the same sharingan eyes needed to be visible in all holes. If one "Madara" has two eye holes with two sharingan, and the other "Madara" has only one eye hole, or two holes but only one eye, Pain would have noticed the difference.
Question: Where did Obito get his second eye?
Answer: After the Uchiha massacre. That's probably where he got his whole collection. Also, this is why the masks only had one eye hole before the massacre, because Obito could have had a second eye hole if he had a spare sharingan before that.
Question: Where did Madara get replacement eyes, after giving his Rinnegan to Nagato?
Answer: I don't know. Some cemetary someplace? Some cadaver during the third world war? Actually, if he was searching the Uchiha bodies for replacement eyes during that war, that's how he could have found and rescued Obito in the first place.
Question: Why did Obito go to Kirigakure?
Answer: He had to go somewhere to hide, while he grew his hair out to pretend to be Madara. At this point, Pain didn't know about Obito, Pain only took orders from Madara. In order for Obito to pretend to be Madara after the real Madara died, Obito had to go hide somewhere while he grew his hair to be the same length as Madara's, until the real Madara died.
Question: When did Madara die?
Answer: According to this timeline, Madara died whenever Yagura died (and whenever Mei Terumi became Mizukage). That's when Obito left Kirigakure to take over as Madara.
Question: How/Where/Why did Madara die?
Answer: I don't know. Someplace secret and away from Akatsuki. Kirigakure is my best bet (we did see Kabuto sneaking around Kirigakure at some point, so that might be where he found the body). One scenario is that Madara traveled to Kirigakure to "recruit a new member" (Kisame). When he gets there he dies, and the "Madara" who returned with Kisame was Obito. Pain never knew the difference.
Question: Yeah, but why Kirigakure of all places?
Answer: He can't go to Konoha, because people there might recognize him. Plus, they've got people there with the Byakugan and Sasuke with the sharingan, and those people would have seen through genjutsu. Amegakure also was Pain's headquarters, and he can't go there if he's hiding from Pain. Sunegakure was too close to Konoha and was being investigated by Orochimaru for his invasion plans. Kirigakure was a place where there was lots of secrecy, where the kage was a child (AND the village jinchuuriki at the same time), and it would have been easiest for him to hide. Why he didn't go to one of the smaller villages, or one of the other large countries, I do not know.
Question: Why didn't Obito go back to Konoha?
Answer: The death of Rin. He held both Minato (the future fourth Hokage) and Kakashi (a celebrated war hero) both partially responsible, and he didn't want to face either of them. We'll know more about this if we ever find out how and when Rin died. I suspect that Obito realized that his sacrifice was wasted because Rin died anyway, and that's when he decided that the whole world was hopeless because hope and sacrifice can be completely wasted by people who are as good as Minato and Kakashi.
Question: Why was Pain supposed to use Rinne Tensei for Tobi?
Answer: To resurrect the real Madara. Tobi wants to use Infinite Tsukuyomi, but his eyes don't have that technique. Only Madara's eyes did. Pain was supposed to resurrect Madara, and Madara uses the technique. When Pain died, that plan failed and Sasuke became the backup plan. This is why Tobi sent Zetsu to make sure Sasuke stayed alive (and why Tobi gave Sasuke Itachi's eyes, to make sure he wasn't blind when it was time to use that technique).
Question: Why the interest in Sasuke?
Answer: Sasuke has a particularly powerful set of MS eyes, with the Amaterasu/Tsukuyomi/Susanoo combination. If Sasuke is indeed related to Madara, he may have the potential to unlock the Rinnegan as well. In addition, Sasuke is Kakashi's former student and doing anything to Sasuke would be punishment for Kakashi. Finally, Sasuke was the backup plan because Obito needed a set of MS eyes to use Tsukuyomi, after Pain died. Obito has the rinnegan now, but probably doesn't have enough chakra to use Rinne Tensei (Nagato, being a powerful Uzumaki, did have enough chakra to use it)
Question: Why does Obito want Naruto and Sasuke to fight?
Answer: Naruto is the son of Minato, who Obito blames for Rin's death. Both of them are Kakashi's students. Forcing them to fight to the death is punishment for Kakashi, and revenge against Minato too. basically, this is just a side-show to the bigger plan.
Question: Why did Obito cut his hair again, when he pretended to be "Tobi"?
Answer: Because Kisame would have recognized a man with waist-length black hair and a one-eyed mask. By cutting his hair short and changing to a new mask design he was able to pretend to be somebody else without Kisame suspecting who he was. This is also why Tobi acted like a complete doofus, unlike what Kisame remembered.
Question: How do we know that Obito didn't massacre the Uchiha, or that Madara didn't invade Konoha or pretend to be the Mizukage?
Answer: Obito had no reason to massacre the Uchiha, but Madara did. Likewise, Madara had no reason to personally target Minato, but Obito did. Obito needed to hide from Pain, but Madara didn't. Madara could have been hanging out in Amegakure in secret, or in some other Akatsuki hideout without problems, because Pain knew Madara existed (Pain never knew about Obito).
Question: When did Akatsuki form?
Answer: There are two versions of Akatsuki. The first is the band of freedom fighters from Amegakure. After Nagato became Pain and killed Hanzo, the second version was set up as a group of powerful international criminals to capture the Bijuu. The freedom fighters from Amegakure just became the normal army of Amegakure after Nagato took over the village, so the Akatsuki name was used for the new purpose with the criminals. At first, the organization did bounty work for other nations as a way to explore the world, gather intelligence, and earn money for operating expenses. After they found all the Bijuu and set up various hideouts and practiced their sealing techniques, they started trying to capture the Bijuu at the end of part 1.
Question: Why does Tobi clinch his arm when talking to Sasuke about Izuna, if he wasn't Izuna or Madara?
Answer: I don't know. Maybe the feeling of giving up your eye for a wasted cause was too familiar to him (it reminded him of his sacrifice to save Rin). Maybe he knew the truth from Madara, and didn't like it. Maybe Izuna is a relative of Obito (grand-father or great-grandfather?)
Question: Couldn't Madara have been the Mizukage, and he used the transformation jutsu to fool Kisame?
Answer: Yes, this is possible. But then again, Madara didn't have any reason to hide from Pain and could have lived comfortably in Amegakure or in the Akatsuki hideout. It's only Obito who needs to hide, because eventually Obito has to pretend to be Madara after Madara dies.
Question: How did Madara live that long? And, if he can live that long, why did he have to die at all?
Answer: I don't know. Old age? Disease? This needs to be explained still.
Question: Why did Madara give the Rinnegan to Nagato at all? Why not keep them for himself and give them to Obito later?
Answer: Madara has to pretend to be dead, because everybody thinks that Hashirama killed him. If Madara reveals himself to be alive, people will try to find him and get involved in his business. For this reason, Madara cannot become the visible leader of a prominent group of famous criminals. Also, we've seen from Nagato that summoning and using the Gedo Mazo requires a lot of chakra and has some risks. Also, the Rinnegan is a source of power and would inspire people to follow the owner. The Rinnegan were required to seal the Bijuu, so somebody visible had to have them. So Madara needed somebody who could be the visible leader of Akatsuki with the Rinnegan. Nagato, who was a young Uzumaki living in a war-torn village, was easy to manipulate. Also, Nagato's Uzumaki heritage meant he had enough chakra to use Rinne Tensei to resurrect Madara.
Question: Both Madara and Obito are on the battlefield now. Which one of them will seal the Juubi?
Answer: Obito appears to be something of a nihilist, so he probably won't care who does it so long as it happens and the world ends. Also, there's no evidence that Obito has Tsukuyomi in his remaining MS eye and Sasuke isn't on his team any more to use or to steal the eyes from. Of course, if Obito wasn't able to cast Tsukuyomi at all, his real secret plan cannot involve using Tsukuyomi, so he might have a secret plan of his own that Madara doesn't know about. I suspect that Madara will seal the Juubi himself, and Obito will fight off the allies during the ritual.

I suspect that Madara will seal the Juubi himself, and Obito will fight off the allies during the ritual.
