Japanese honorifics are things such as "-san" "-sama", and many others. There is one which is often translated as "uncle", though it does not mean biological uncle. I believe another way of translating it is "old man", but because that's not as respectful in English as the phrase is in Japanese, it's rarely done this way. In a sense, he's really saying "old man second", or something to that affect.

That's just how he identifies people:
Grandma Tsunade (Tsunade)
Geezer Toad (Fukusaka)
Chief Toad (Gamabunta)
and I guess Uncle Second (Tobirama)
