for two main reasons

1.It was Naruto's dream to become the Hokage from day one so killing him off without him becoming Hokage is pointless to the series.

2.Sasuke gives a rat's ass about a Hokage or any Kage title for that matter.

well the story probably wouldn't continue to the point of narutos death. it would be between sasuke, shikamaru, and konohamaru depending what kind of ninja konohamaru became. shikamaru was the apprentice of the third hokages son and very intelligent and fit to be it. sasuke is fit for it as well plus the connections and his ability he can do the job.
