Vegetative state is basically being brain dead, unaware of anything. The fact that they no longer have control of their body and are incapable of resurface basically means they're worse off than being dead since their soul is in a state of suspense.

He technically did murder and experiment on people to achieve his immortality though.

Meh powerful jutsu come with a price. Technically Tsunade is how we have seen her the entire series, old Tsunade hardly appears and we've never seen the face.

Her transformation jutsu is different to the usual, as it does not disperse when she is damaged, only when she is on her last drops of chakra.
