Chakra scalpels don't help if his chest is missing.

In that case, Tsunade effortlessly destroys part 1 Kabuto. He was only a high jounin level ninja yet Tsunade managed to hold her own against 5 susanoo clones, any of which would destroy part 1 Kabuto.

Kabuto had difficulty avoiding a rusty Tsunade suffering from a hangover so current healthy Tsunade will easily hit him and kill him within the first few seconds the battle starts. Also, part 1 Kabuto doesn't have the strength feats in decapitating Tsunade with a chakra scalpel. Also part 1 Kabuto's chakra scalpel is vastly inferior to DSM Kabuto's chakra scalpel.

All in all, current Tsunade wins low difficulty with or without Katsuyu.
