Itachi cancelled out Sasuke's Katon which is much larger than Tsunade's body. He can make flames just as large as Sasuke who fried the 8-tails, he just can't manipulate them as well. Look at these flames in comparison to Naruto's body.

Itachi can not take this low difficulty. Yes Tsunade may be on fire but she can also force him into removing it because if she hits him while she's on fire he's screwed too, especially if she keeps coming. The point about Amaterasu however was meant to counter the eventual he will just cover his Susanoo with Amaterasu. She can counter most of his abilities. Without Yata Mirror and Sword of Totsuka she poses a real threat. He wins but with high difficulty.

Yes he has amazing abilities but it seems Senju serve as a natural counter to most. The deciding factor in Itachi's victory is his Sword of Totsuka and Shield.
If we ignore the Itachi fanboyism for a bit we realize other characters are not simply fodder before him.
Itachi wouldn't use a full scale Amaterasu on her because:

If you think well Susanoo will protect him, the flames that were on Sasuke's susanoo were not attacking it but as Mei's lava showed, Susanoo melts and if the black flames are attacking and the flames are hotter than Mei's lava Susanoo will melt and Itachi will still be screwed. Amaterasu suppression seems like it takes a lot out of you based on Sasuke's response when he fought Bee. You may argue she'd be in too much Pain to fight back. But I don't think so. She was completely impaled and continued to fight, 3 yasaka magatamas at point blank range didn't stop her, nor did hemophobia in conjunction with the sword of kusanagi's three wounds and Kabuto cutting her abdomen, respiratory, and bicep muscles.
When you add in the fact that we know Susanoo is weak to acid (as demonstrated by Mei whose acid wasn't melting surrounding rocks) and we know Tsunade has a summon with acid strong enough to melt through rock, and we know the summon can split herself many times enough to cover konoha, there is no reason those smaller katsuyus can't launch a coordinated acid attack that would melt Susanoo. And we know that a melting Susanoo is easier to break through.
This will not be an easy fight for Itachi.
She cuts off her arm when she gets hit with Amaterasu? If Itachi use Amaterasu, he hits her whole body and she's fucked. Tsunade gets from Itachis full abilities, Genjutsus, Amaterasu and Susanoo, no way for her to counter anything.
For your information there are ways of countering Amaterasu. There is a fire sealing scroll that Jiraiya had which could seal amaterasu and because amaterasu burns very slowly it could be done with relative ease. Also Tsunade need not cut off her hand at all she has the Byakugo which would allow her to heal every wound or burn instantly. Also she has almost double of Itachi's stamina and durability. I'd like to see Itachi tank Konoha size Shinra Tensei from close quarters and without healing. Where speed is concerned He is faster but she has good reflexes and better summons. Also she bests him in physical power and Taijutsu. Not that I belive Itachi would be dumb enough to attempt a Taijutsu battle with her. As for Tsukuyomi she has been seen to heal the effect of Tsukuyomi on others, so why not on herself? And as for Susanoo, she's already shown that she can crack a Susanoo with a mere punch and kick.
