She's not immune to all Genjutsu, Tsukiyoumi would deal with her. Amaretsu is completely discounted, not to mention Itachi's own intelligent fighting style.

Without or without Susann'o, Itachi wins this match up.

Amaterasu isn't your regular everyday Katon. She can't just tank it - she needs to somehow remove the flames from her body, because she's not quick enough to dodge them. Even if the time it takes to completely incapacitate her is noticeably longer (compared to the average person) because of her Byakugō, the flames will eventually overwhelm her. You also need to realise that she'll be in constant agony whilst engulfed by Amaterasu, and as a result, is probably more likely to make mistakes or miss the minor details against Itachi, allowing him to easily pick her off.

Itachi takes it with low difficult at best if I take a look at it realistically. Tsunade isn't really versatile, her fighting style is limited to close combat and her medical techniques. Fighting an Uchiha of Itachis level with Taijutsu would be virtually impossible as Itachis ability to foresee movements would allow him to counter her techniques with his mere speed. Already Sasuke, whose Taijutsu and particularly speed was inferior to Itachis, handled dozens of Shinobi at once without they left a single scratch on him when training with Orochimaru. Itachi is one of the most versatile Ninjas, he's excellently at using all Ninja arts such as Ninjutsu, Taijutsu and Genjutsu, where latter is as good as always sufficient in beating someone with a single usage, examples are Deidara and even Orochimaru. He's not only a Genjutsu user but makes also use of tactical feints which are used with a speed the opponent can't even follow.

Restrict the Mangekyou Sharingan as a whole and Itachi will still beat her with moderate difficult.

If he wants to defeat Tsunade in a taijutsu battle, he would loose. A taijutsu battle with Tsunade is equal with suicide.
