But using Susanoo and Ama (two jutsus that tire him the most) at the same time would drain him a lot. Even a 30% Itachi was able to perform the said Katon without visible straining, but wasn't able to use either Ama or Susanoo alone, much less together.

Once Tsunade is dead this fight is over.

And then Susanoo for chapters 392 and 393 and the beginning or 394.

Yet you think he will collapse and cough blood, from using 2 smaller scaled Amaterasu's and a lvl 1 Ribcage Susanoo when he was using a full lvl 4 one before? No, that doesn't make sense.

Not to mention 1 Tsukuyomi can just wrap this up quickly. Itachi's sickness is never becoming a factor here.

Either way, it all comes down to whether she can avoid the Amaterasu or not, something I must say is doubtful. The people that say she can tank the Amaterasu are being ridiculous.

He has Sage Mode, Frog Summons, Rasengan, and Katon, along with hir Hair jutsu and Swamp of the Underworld. His abilities outclass hers, and by a fair margin at that.

Nevertheless, only Zetsu and Nagato characterized the first usage of Amaterasu as an air pressure rising from the heat, when both were sensors. A on the other hand was orientating by the mere activation of Sasuke's Mangekyo Sharingan. Having a very one sided fighting style, Itachi wouldn't have problems in predicting Tsunade's actions and same time counterattacking within the large openings she leaves. To clarify how skilled Itachi's analytical skills are, I'll just post 2 scans (used by Blaze before):

Kabuto notes that Itachi even exceeds the talent and power of an Uchiha standard significantly, being able to read the opponent like an open book.

Full knowledge about Itachi will not prevent her from being overwhelmed by Itachi's Mangekyo Sharingan. Facing Tsukuyomi and Amaterasu at the same time makes it as good as impossible to survive, as one eye contact already ends the battle such as a glance does. Trying to avoid eye contact to obviate Tsukuyomi, even those whose speed is great enough to dodge Amaterasu will become a perfect target, as another way apart from predicting Amaterasu via eye contact is not possible. One blow of Totsuka would also either kill or seal her, quite to Itachi's desire. Being larger, faster and stronger, it's to be assumed that Tsunade doesn't have a way of dodging formers attacks.
