She is perfectly capable of the title hokage. She excels in medical nin and is referred to as the strongest kunoichi by Shikaku. She changed Sakura who otherwise would have become a fail product. Sure Combat wise she is lacking but to me that doesn't determine true strength. Her ever-willing determination to protect her village is a true hokage strength.

tsunade will always stay like that, because people tend to compare her with the likes minato hiruzen or harishaa, as a kage i will sorry for her
