Only times medical ninjutsu was offensive was with: the chakra scalpel which needed precision to be used for fatal blows in battle, whatever jutsu tsunade was going to use on Orochimaru in part 1, and the one hit wonder of body pathway derangement. I may be missing some others but medical ninjutsu takes a supplemental role in the grand schemes compared to ninjutsu, dojutsu, genjutsu, and arguably taijutsu.
