Medical Ninjutsu is the ONLY field in which Tsunade has an advantage over Hiruzen. She is better suited to a team or squad battle, while Hiruzen is capable of going 1-on-1 and working in a team. Overall, Hiruzen could wipe the floor with Tsunade. In a healing contest, she blows everybody else in Konohagakure out of the water....for now.

tsunades revealed herself as the third strongest hokage IMO. she was compared to raikage in speed (if she was slower by a huge margin, why would madara bring it up?) and is possibly physically the strongest and most durable character besides third raikage.
katsuyu is an absolutely beast summon. division for defence, acid spit for offence and insane durability for her own.
