Tsunade accelerated Shikamaru's recovery by transferring chakra to him.

She didn't preform mystical palm, emergency treatment or preform anything medical.

She just transfered chakra.

Sakura extended her Byakugou seal onto Obito.

Fair play to you, it's a pretty canon-friendly (& reasonable) excuse; Sakura failed to react.

Not that I think it applies here, but still... I can see why went with that, it's something she's been doing since Part I on a fairly constant basis.

It doesn't explain why she couldn't transfer her own chakra - if that's all that was needed. Surely she had enough of her own, what with the Seal and the Kyubi cloak/chakra.
