Think about it, the world govt saw a child like him who took his fathershead too marejois . No normal kid would do that. Even so, doflamingo managed to survive. Hence they sent down a agent of their own undercover wearing a muckus costume.
Trebol can be seen/thought to have been the older and more intelligent one even in the flashback in this weeks chapter. He has stuck by Doflamingo all this time, and even in his demise he stayed back with doffy whilst he others left. Maybe the costume or whatever it is that covers his skinny appearance is just a disguise.
Will not be suprising, as in the movie 3D2Y, Worlds own crew CIpher pol were disguised as pirates then nekminnut they turn against World and his crew when marines attack Worlds ship. This can be reinforced with the govt being able to keep tabs on doflamingo becauseof the secret he has on them. Maybe corazon wasnt the only traitor?
Heck even his face looks like one of them masks that cipher pol generally wear.
