Orochimaru was just not paying attention during his monologue, as villains do, and also wasn't worried about getting stabbed because he thought it was a regular susanoo sword.....he had no knowledge of it being Totsuka.


Nagato had lack of mobility to dodge it, and also, Itachi got rid of his field of vision which created a smokescreen.

This was all stated in the manga


If Itachi and Totsuka were so godly, then he would have been the main hero and stopped akatsuki single-handed. If no one could sense totsuka, then he could just have snuck up on them and sealed them without them knowing.....but he didn't because he can't.

Orochimaru was caught mid-sentence so it might be very quick (or he couldn't do anything about it since well, he was practically stuck in his snake's mouth. Immobile) and was still rather arrogant after being stabbed thinking it was an ordinary sword. Which was a mistake on his part.

Itachi took advantage of the smoke to stab Nagato and Nagato wasn't able to sense it because it's a weapon, not chakra. Or should I say, Kabuto.
