1. Kushina gave birth before she died... birth alone is the worst
2. Itachi suffered for ages before he was finally killed
3. Kisame was eaten alive by water sharks
4. Hiruzen drew out his death as long as he could, suffering from the amount of pain he was going throw yet died with a smile on his face
5. Jiraiya took so much damage facing pain before he died

Emotion was not a factor I listed over the amount of pain and suffering that they went through.
