So Chapter 78 happened in the past apparently is wasn't occurring the same time as everything in Cochlea. The body Urie saw was actually Torso's dead body. It looks like a minor character from the CCG named Tokage came and eviscerated Torso and saved Mutsuki. Also it looks like Mutsuki has her arms back but I'm not sure about her legs. English chapter should be out in 4 hours or less.

Ok so the body was torso's, and Mutsuki is a deranged psychopath who murder things subconsciously. Nice one Ishida-sensei, not bad at all.

For some odd reason the special class & his Master were Kaneki & Arima. That's really the only duo I can think of. Maybe I'm forgetting something, with the sheer amount of characters that come & go in this Manga i tend to forget names if they're not present enough.
