This design shows maddy EMS which had no special power.
Shisui's eye is very OP MS contesting with kamui itself.

That's because Madara still had his ms.

Essentially the battle against Hashirama was to gain senju power over his brothers eyes.

Once he gained his cells, he implanted his ms which became the rinnegan.

So that spare eye he had in the cave was his last left EMS eye.

Obito doesn't have that option
