Remember when shukaku's sand easily entered madara's skin? Well its like that, however tobirama's spit was aiming for madara's head & it was at point blank range. If that spit entered madara's brain he would most likely be dead [implying that tsunade can't survive brain damage], which is why he used a simple yet effective susanoo hand [the smallest form of susanoo we've seen thus far] to block it.
