3. Spiral Zetsu (which is different from normal white zetsu)
4. Madara + Obito
5. Obito + Spiral Zetsu

It is obvious that Madara was using Obito's body through Spiral Zetsu.
It's obvious that the retarded version of Tobi was Obito's continous thoughts of knowing that he kills people and does wrong by following Madara.
It's obvious that Obito became Obito when he ditched spiral zetsu and dressed in that awesome 3 tomoe sharingan grey mask and those purple clothes.

This change is extremely important in Obito because he changed after the fight with Konan when he learned that Nagato used RinneRebirth for Naruto and not for Madara.

So => Obito in spiral zetsu = the time period when Obito was under Madara.
