for the same reason kakashi wears a mask... its in his character, he probably wont feel the same without it. but for madara i think its because he wants to keep an intimidating appeal and what best way to do it but have mask to hold secrecy to throw off his opposition!? besides , madara probably has a messed up face/body from all the work hes done to it by trying to figure out jutsu like izinagi etc.

1. Maybe he just has immense chakra so that it would be a waste of time for him to always change
2. He has a mask so it's difficult to see that and btw: maybe he's just so strong he doesn't even need to activate his MS/EMS... the fight with Konan was just a shocker he clearly underestimated her
3. That is still unsure we still don't know why he has a mask on/why he covers his left eye
