Ok so how did Madara know about Nagato who was born way after his death? Simple Nagato's his grandson.
Here's what I think:
Madara had a son before his fight with hashirama and that son was Tobi. At the time of hashirama's clash with Madara, Tobi was probably in his early teens. Which is the rreason why Tobi knows a lot about Madara(his dad) and also share the hatred for konohagakure and which is why he helped itachi take revenge against the uchihas because they had abandoned his father.
Madara knew that he was going to die from the injuries that he had sustained from the clash with Hashirama so he gave his son(Tobi) specific instructions. He told him to take his newly aquired rinnegan and implant it in his future son(and I'm guessing they discussed the son's name to Nagato since upon Madara's ressurection he knows of the name Nagato). So at some time after Nagato's birth Tobi implanted the rinnegan into nagato and hid in the shadows to surface at the start of the series when he attacked konoha with the nine tails.
Tobi's wife was an uzumaki which would lead to nagato being able to use rinnegan from birth because he already has senju dna in him (uzumaki related to senju).
So yeh what do you think Narutobase... I think this is more likely than izuna, simply because izuna is way too old and i think this would make for a great twist in the story.
