there is a difference madara's eyes had no light he had lost his sight so he was blind from over use of the mangeyko that is why i reckon he couldn't just swap them they where gone they would have been useless to izuna as they where blind .

where as itachi wasn't fully blind he was blurry at worst and take into account he had just came out of sussano and had his disease so being slightly blurry is pretty good compared to being Fully blind therefore useless to trade where as itachi's eyes where still usable and perfectly well for trading them .

no. when madara got izunas eyes, he didnt expect to get EMS, he just wanted some eyes that werent blind. he probably didnt even realize that these new eyes dont get blind until a while later. madaras eyes were completely blind so izuna obviously didnt wanna bother transplanting eyes that dont even work.

i just dont get why izuna didnt take someone elses regular sharingan or byakugan or even just regular eyes

We know that Tobi fabricated some parts of his tale (the Nine-Tails attack) to fool a gullible Sasuke into a revenge-quest, whose to say he didn't lie about Izuna/Madara's history?

If we assume that Tobi is lying about the two brothers history, then obviously something (the truth?) made Tobi so upset as to clench his arm. Perhaps what really happened between Izuna and Madara was so horrifying that it turned him into who Tobi is today.
