Kishi put his disease in last second just so that Itachi would die after he fought with Sasuke. It was a very last minute thing, we don't even know what the illness was.

1 he probably wouldnt get susano just all rinnengan abilities...since susannoo is said to be a rare ms ability....(itachi was surprised to see sasuke with it)....and its just luck that most of the ms users we have seen have it.....

2 if itachi wasnt sick and actually killed sasuke and took his eyes then he would be close to some of the moderate fanboys of him(pretty op) but since he was sick ems wouldnt have done a whole lot...since he rarely used ms abilities anyways

3 itachi would be more op with renningan(without sickness) because itachi uses a ton more strategy and is more precise than sasuke....if itachi sick then sasuke....

Well you need tsukoyomi and amateratsu to activate Susanoo. So no, Obito wouldn't get it since he has none of these attributes, Obito has stated himself that it is very rare for one to unlock it.
