what i believe happened was that once you activate the MS and use the eye power you can be able to use it without the eyes.

that or Itachi and Madara were so amazing that they learned how to perform the jutsu without their sharingan, they are that epic.

Lol, shouldn't this be questioned as well? Anyway, prior to summoning Susano O he did have an active sharingan, so you could also speculate that he summoned it, lost the light and just kept it going since it was his final jutsu. Just as valid IMO.

What all of you Madara fans are forgetting with this half assed explanation is the fact that whether or not Itachi was blind atleast he HAD eyes to begin with.

Madara is using a sharingan jutsu without a bloody sharingan to draw from.. thus quite simply it's one of the many Madara plotholes.

It's like shooting bullets without a gun..
