Keep in mind this is just a theory.
As we know the sharingan is developed by an Uchiha's emotional extremes.
The intense emotions create hormones which effect the eyes so that they have the abilities you all know.
The way this works is that the emotional stress manifests a large tumor in the eyes.
This tumor is a modification of the eyes and if too much emotionally charged chakra goes through the eyes then,
although this makes them significantly more powerful, it also increases the size of the tumor creating a
condition called Haemolacria.
"Haemolacria is a physical condition that causes a person to produce tears that are partially composed of blood. It can manifest as tears that are anything from merely red-tinged to appearing to be entirely made of blood."
"Haemolacria is a symptom of a number of diseases, and may also be indicative of a tumor in the lacrimal apparatus."
