This is a theory I just thought up while browsing some episodes..
Orochimaru gave Itachi, the illness in order to absorb his body.
Oro is absorbed by sasuke in episode 114.
Immediately after sasuke forms a team in the following 3 episodes(115, 116,117) and heads for itachi.
I believe this was all planned by oro, as he knew of itachi's illness and knowing so basically flat out gave sasuke a motive to kill him, probably based on some speech he gave him i.e "Your more powerful than itachi now, blabla.."
Sasuke soon after forming the team, tracked down itachi after dealing with issues along the way. Near the end of the battle, sasuke had used his strongest technique on itachi and itachi was forced to use susano'o. This drained himself of basically all the life he had left.
At this point sasuke is out of chakra and oro absorbs him, he now believes he is able to absorb itachi, the stronger of the two brothers. There is some dialouge in between the battle which states oro couldn't be defeated by susano'os attacks which meant defeating itachi would have been easy. Little did he know he had the totsuka blade and sealing gourd thing..
oro also stated that he had no idea Itachi had them in the first place, which meant he would not of planned for this, duh..
In doing this he had not only 1 option for his next body, but two & they were both uchihas!
in conclusion and based on the data above, we can conclude that oro probably gave itachi the illness, that is why he was aware of it when nobody else was, not even the akutski.
