And even then, Itachi consciously used KA on himself and it still worked.

Yeah but if you know your opponent has KA and how it works, shouldnt you be able to get suspicious of your own thoughts if they are odd?

for example, lets say he uses KA to get itachi to kill himself, shouldnt he be able to realize that those thoughts come from the KA? because he would never want to kill himself otherwise and he KNOWS how KA works

Koto Amatsukami is like inception, it is a thought placed in your subconscious that you act on unknowingly. How would Itachi know if anything he was thinking about doing is his idea or Koto Amatsukami, either? He may actually start second guessing himself and making mistakes in battle when Koto Amatsukami may had not even been used yet.

That may be a poor example because Itachi was suicidal and was goading Sasuke into killing him. Koto Amatsukami will give Itachi the subconscious idea to kill himself, and his conscious mind will rationalize it as if it were his own idea.
Then again, I don't know if Koto Amatsukami can actually be used to make someone kill themselves. That may be a logical extreme of the jutsu's mechanics, but not necessarily how it works.
