There have been too many threads about this subject and to this day, still pop up and are used in arguments
it seems that most people don't understand how Izanami works, and due to that misinformation, they believe that Izanami is a one-shot tech.
So here is a detailed explanation so we can finally conclude that Itachi died multiple times in real time before Izanami went into affect.
Now it is debatable whether or not he could have still pulled it off if he was a non-edo, but fact is that he did indeed get fatally wounded multiple times before Izanami
First off let me start of where Itachi says that he activated Izanami when first got stabbed by Kabuto
Activating Izanami doesn't mean it automatically casts genjutsu.....you first have to repeat a certain pattern to fool the senses so that you can close the loop.
Itachi first activated Izanami in the first stab......it has no effect until the movements are replicated in order to deceive the senses, and that happened once he got stabbed the second time....
when it is first cast, it is only in recording mode until the loop is complete
Itachi clearly says it right here.....look I've even circled it in red in the posted page
"replicate the same bodily sensation intentionally"
the intentional loop, meaning the loop that was not in the mind/genjutsu, but manually replicated to complete Izanami and finish its "recording" so that it could repeat..
which means, all those deaths that happened before the loop was complete, happened in real-time and were not an illusion
Here is when Izanami is fully complete and starts to take effect/cast genjutsu, which is why Itachi loses his eye after the second stab(because he completed the pattern)
Kabuto was still missing his horn throughout the whole replication process only until the loop was finished
Itachi died multiple times before the loop was finished and you can clearly see Kabuto still missing his horn
So to conclude and summarize what I said
Izanami activated when Itachi was first stabbed.....it was only recording the process, not repeating it
Izanami has to record the intentionally(keyword from Itachi's quote) repeated pattern for it to be able to loop
once that pattern is manually repeated, as in having to do it for real and not in a genjutsu
only then will Izanami be complete and replay the actions over and over again
Itachi says that he has to repeat the pattern in order to close the loop

Itachi died 3 times, and was caught in a genjutsu with Sasuke before Izanami had any effect....so there you go people
