This is my own theory on how the sage got the Rinnegan.
Some people believe the sage was born with the Rinnegan or gained it before he sealed the Juubi inside of himself. But that's only speculation... It was never stated how he gained the Rinnegan.
We have seen Madara awaken the Rinnegan by fusing himself with Hashirama's dna. It's totally plausible to think that the sage gained the Rinnegan by blood rights, because the manga said that his sons were born with his powers and when you combine those powers, you get the Rinnegan. But some are forgetting the Juubi. We have seen how Jinchuuriki get influenced by their bijuu's and gain their characteristics and abilities.
For example:
Naruto get's Kurama's eyes when he is influenced by his chakra and gain the ability to heal and now sense evil intentions.
Let's look at the Juubi's eye:
The Juubi has a Sharingan/Rinnegan combo. You can argue that the sage didn't have tomoe's, but I will explain why soon...
This is how I think the sage awakened the Rinnegan:
We all know the Juubi is made out of a mass of natural energy. Kurama explained it to Naruto:
Now my theory is.. Because of the massive amount of nature energy supplied by the juubi, the sage awakened the Rinnegan.
We saw how Naruto get's Kurama's eyes when he gets influenced by his chakra; it is also plausible that the sage gained the Juubi's eyes. But even though Kurama is a portion of the Juubi's chakra, they have different chakra natures. So, because the Juubi is a mass of Natural energy, it's only right that the sage gained a type of SM influence and just like how Naruto gains toad eyes, the sage gained the rinnegan. Because of him having a huge amount of Natural energy inside of him, his SM became permanent.
The only thing that doesn't fit is the tomoe's. My theory is that it's the Juubi's influence that's manifested into the tomoe's and because the sage is a loving man, he suppressed the Juubi's influence completely(Mito explained that love supresses a tailed beasts hatred). I think the rinnegan and tomoe on his back symbolize this:
For more proof that the tomoe are the Juubi's influence, Let's look at the sharingan:
Tobirama said that the Uchiha awakens the sharingan when they lose a loved one and special chakra gets spout from their brains into their eyes. The sharingan looks like a lesser version of the Juubi's eye, so it's safe to assume that it's the Juubi's chakra being spout from their brains. We know that the Juubi's chakra is natural energy, so It's actually natural chakra that's being spout from their brains. But that's not all they gain, they also gain hatred and as the hatred increases, their eyes becomes stronger.
The first stage of the sharingan gets stronger when the tomoe increase and then they need to lose a loved one to get the MS(which evolves their tomoe's), to which their hatred increases so much that they lose sight of who they once were. When they reach EMS(By implanting another users MS into their own eye sockets, their MS tomoe's combines), they become like madara and go completely into darkness. Do you see the pattern? Every time their tomoe evolves, their hatred increases. I believe the tomoe's is the Juubi's influence.
Now I'm going to prove to you why It's natural chakra spouting from their brains
We know that the older son was born with the sage's eyes. Let's look at his eyes:
Look closely and see that he has no tomoe's and he has the same markings on his face that Naruto and Hashirama has when they enter SM. The reason for his lack of tomoe, may be that he gained his hatred after this pic.
Sasuke was also compatible with the CM, which works by collecting natural energy. The Uchiha weren't known for their high chakra levels and yet they are compatible with natural energy..
So it may be possible that the Rinnegan may have manifested by the massive amount of nature energy the Juubi has.

Have you noticed how the rinnegan affected Madara. He went from trying to destroy everything to finding world peace. It's just that he has the same beliefs the older son of the sage had, to find peace through power (That was before he got angry at his brother). At first madara wanted to live peacefully with the Senju, but after achieving EMS, he didn't care and wanted to destroy everything. Now that he has the rinnegan, he suddenly wanted peace.
Obito and the rinnegan
Obito, when he was Tobi, was different. He seemed truely evil. After he got the rinnegan and was unmasked, he suddenly changed and wanted to listen to Naruto's answers. Tobi wouldn't have done that.
In a way, the Rinnegan supresses the Juubi's influence or the body of the sage supresses the Juubi's influence and then the rinnegan is awakened. If Obito is changed by Naruto, then that's proof that it really is like that. Madara was always ignorant, so he won't come to his senses so easily.
