All I know is whoever it is we are gonna probably b up to our noses in flashbacks to explain/give insight into them.
Options and my analysis
1. Kabuto:
Is debatable now coz of Karin not recognising the chakra coz if he cld escape the loop he wld b good so not possible to have that sinister chakra, though let me take a shot in the dark- its Kabuto's body but when Oro took his chakra back it disrupted the balance in Kabuto so maybe he under went an effect of failing to control nature energ but coz snakes are trecherous the Snake Sage takes over yo body instead of u turning to stone, thr Snake lord wld break out coz if someone is that much attuned to natural energy and nature they can see thru illusions and differentiate btwn them and the Real world.


2/3. Known but hasnt been active charecter:
Shisui- its quite possible but I dont know what kinda impact he wld have on the story seeing as this is the elevenght hour and unless Kishi is planning to have Madara defeated in 2017 Shisui n similar charecters bring in too much coz there is the how he survived, whats he been doing, what and how in regards to their power then finally why he is saving Sasuke(last one more pointed to especially the guy whi saved Madara or whatever the story is on that )

4. Complete new entry
Dependent on their flashbacks then manga ends 2017 in the very least for sure.
Ps Im not a narutologist just a hypocritic and I got bored typing sol left out stuff in that.

Its obviously a user of Sage mode, but who and where they came from is a mystery. If they emit such bad feels, enough for Karin to pick up its certainly not any user we know of currently unless Kabuto managed to somehow escape Izanami with his evil motivations intact, in which case Orochimaru would now who it is.

As said, the sage mode, the modifications to his dna and maybe the izanami too, could have changed his chakra signature.

Sasuke his chakra became different as well, when he became darker and colder.
Karin at a certain time, found it almost unrecognizable. That is how much it changed.

Chakra signatures can change.

Mysterious feet.

The person who is next to sasuke could be the same person who saved Madara at the VOTE. Like an Uchiha guardian angel, coming to the aid of dying Uchiha's jk. Karin stated she has a very bad feeling about this person, im sure the person who saved Madara isnt a good Samaritan who helped Madara out because he was a good boy, he would be a villain with evil chakra Karin would notice. It would also reveal how Madara was saved, if he was saved by someone. its a far fetch idea tho.
If Madara was stabbed thru the chest with a sword and somehow survived, im sure Sasuke who also got stabbed thru the chest with a sword will survive the same way.

For those who say its Kabuto, it wouldnt make since seeing how to escape Itachi's Izanami he would have to accept himself and what not becoming a good character, not giving Karin a bad feeling. For those who say it his sage mode, Karin wouldnt be able to sense him at all since its Nature chakra, just like how Naruto had to go sage mode in order to sense the Juubi.

How hilarious would it be after all those guesses of tobi being Izuna, this dude is him XD

exactly because this is Kabuto but kishi uses all mind tricks to make us confused while making from obvious reveal great twist. That way to go Kishi it always work for these who not pay attention to details which give us hints or can not see bigger picture behind manga reveal and plot advance.

The trick Kishi used here to pull us away from obvious answer being Kabuto are two things:

- Karin didnt recognize chakra of mysterious man and also she called this chakra terrible
- the silthouete of this person was shown with something like horns growing from head and spikes on back/shoulders.

Kishi plays very smart to prove Kabuto being out of question but let take while and wonder what people with horns and spikes we know .

- Rikudou sennin(deceased)
- Current Madara
- Obito used to have them as ten tails host
- dragon/ snake sage mode Kabuto - BINGO this is reply to our question

Dsm kabuto not just cover silthouete form with horns point but also Karin description as ( karin never met kabuto in snake sage mode also dsm/ ssm was revealed as the most evil sage mode so it would be another hint and reference.

It completly fits plot advance as big reveal as It would shown Kabuto finally accepting himself and leaving izanami. Also Kabuto would get chance to reedem himself and return Itachi's favour by saving Sasuke's life.

Also funny that some people call him weak while he was appearing at 6/7th place in top 10 constantly also taking in consideration that rest characters besides Madara and bz obito are out of picture in the moment so he would the most powerfull shinobi on good guys side + being the biggest asset as he sould be the only possesing sage mode used to fight obito as juubi jin.

Conclusion: Kabuto is the most logical and sensefull choice to advance plot while bringing not just new stuff but also bring more deepness to manga and his character. Kishi always build up tension and set strong hints using obvious answer but utilizing all psyhological/mind tricks to fool most of people and make the obvious answer great post twist.

Believe it or not but mystery man is noone other then dragon/ snake sage mode kabuto. Kishi make us eliminate kabuto out of possibilities as most people forgot or think kabuto has no longer dsm/ssm so we compare using base kabuto.

You do remember that for Kabuto to have Sage Mode he needs Orochimaru's and Jugo's chakra. I'm not saying that Kabuto is the one next to Sasuke, but it certainly isn't Sage Mode Kabuto.

He may have Juugo's power but remember what Fukasaku said? Sage energy will eat you up unless you already have a large amount of chakra. Orochimaru's chakra gave Kabuto that large amount of chakra. Without that, Kabuto can't easily take in sage energy and he would probably be dead by now. So it might be Kabuto, but not DSM Kabuto

Kabuto has Juugo's and Karin's power. Karin is an Uzumaki. Kabuto has plenty without Oro's power.
