Whoever this John Doe is , he was the one who implanted and manipulated Hashirama's DNA into Madara. Here is the situation in a nutshell;
• He was watching the entire battle of Hashirama and Madara
• A very skilled individual at Medical Ninjutsu
• He was the one who collected, implanted and manipulated Hashirama's DNA
*Its said this person was an mere acquaintance of one of Madara's clan underlings, meaning this person might not be Uchiha at all!*
Everything from here on is just speculation;
The Lab
The Mountain's Graveyard (Madara's Hideout) might be a place where this mystery man practiced his experimentation, supposedly on summoning animals to create "The Ultimate Beast", which may explain the multitude of giant bones of various animals.
Secret Tunnels
I honestly don't know whether Madara or the mystery man created these tunnels, but it seems to span nearly all over the world, even to Kanabi Bridge, where Obito died, and where Madara found him in his hideout's cave.

These caves must have gone everywhere, the Zetsu Army even used these tunnels to attack from under against the Shinobi Alliance if I can remember correctly
