Not always. Yuske lost quite a few battles, Goku got killed a few times, Kenshin Himura lost against Shishio, Ichigo lost to Juhabach, and I can keep going. If you want more older Shonen I have even more examples. Kishi writes cliches, so I think Naruto wins and doesn't throw it like he did the first time when he went after Sasuke's headband vs getting a direct hit.
