Madara is the one who said that whoever sees the perfect susanoo will die. Consider that he is the one who developed PS, and that there doesn't need to be another user out there. Concerning Hashi, it's possible that Madara didn't have or use PS in their fight. However, Madara could have also just been trying to intimidate the kages with his assertion that everyone who has seen PS dies.
