I really want to know who will when in an all out battle.

Both at they're natural peak (EMS Madara v Pt. 1 MS Itachi)

Who wins and why?

No totsuka blade & yata mirror

Madara would beat Itachi. Madara would be able to break out of tsukoyomi and his power is overall greater than itachi.

The deciding factor is really Perfect Susano'o, as I believe they are pretty equal in taijutsu. Genjutsu is irrellevant.

Healthy Sasuke was getting tagged by a sick sluggish itachi. We NEVER saw his full skill and yet it was the best taijutsu battle in the whole show. I'm using that to put him on Madaras level.

Perfect Susano'o is definitely the deciding factor. Only trolls would seriously argue that Itachi's Yata mirror can actually block it. Any literal interpretation that the Yata Mirror magically blocks anything is like saying that Hiruzen can literally use every Konoha Jutsu (Tsukuyomi, 8 Gates, Shinsusenju, etc.).
