I haven't once seen the Yata mirror protect against a significant attack and the one time the Totsuka Sword was used successfully was via a sneak attack.

Depends on which version of these Uchiha's we're talking. Itachi is strong obviously, but if it's by the end of manga, unfortunately for him, other Uchiha's achieved godly levels(JJ Obito, JJ Madara, Rinnegan Sasuke) and one of his strongest techniques tsukuyomi won't work on god level Uchiha's.

No one around was strong enough to take JJ Madara. So Kishi killed him with Plot no Jutsu. Doesn't change the fact that JJ Madara shit stomps your beloved Sasuke.
