She's referring to when sakura removed the poison from kangkuro , she used water to contain the poison within a water bubble of sorts.

Definitely Itachi. Pining for anything above that is a bit much but Kisame in the water puts him at the absolute top of high tier along the likes of stronger kages, higher-up Akatsuki and Sannin. Out of the water, he pushes Itachi but his bigger guns will extinguish Kisame.
