She can only appear young[

] with the use of her seal's chakra. Key word 'look' meaning she was able to accomplish all her feats with a very old body.

Now pretty much everyone can do better with a younger body but with what Tsunade is able to do with an old body, I dont even want to think about how powerful she was when she had a younger body.

Basically put, her Creation Rebirth ages her. We grow and age through cell division, but that's also the same way we heal. When we have a wound, cells divide to close it up. However, the body has a finite, set number of times its cells can divide. By forcibly causing them to divide with Creation Rebirth, she causes her body to age more rapidly.
