So why exactly isn't Gai's leg healed up yet?

10 to 15 years down the line of Naruto becoming Hokage, and he is still in a wheelchair. I understand that kick to Madara shattered his bones, but didn't Tsunade not reattach one half of her body to the other (which is miles worse than Gai's leg) and create a prosthetic arm for Naruto? If this has something to do with 8 gates premantely damaging the healing ratio of his body, or he doesn't want to be healed and counts his leg as a mark of valor, I understand. But I don't see why he couldn't be healed based off what we seen.

I feel like he just accepted that his youth is over and that he doesn't need to work for others now as he already gave (or intended to) the ultimate sacrifice

I was thinking it was more of pride thing too, maybe we will get more info in the upcoming mangas, I mean yeah, having the pride to admit you're done is great, but wouldn't you want your leg back?

Or you know. Some wounds can't be healed. He completely shattered his leg. Every nerve, fiber, ligament and bone sustained huge amounts of damage. People end up crippled for less severe injuries

who knows probably plot for sasuke to make his appearance in the last if you look at it if gai was heal in the last he would have destroy that meteor with the hirudora

Tsunade's spine was in way worse condition, and she healed that up in a matter of minutes. If his leg is that bad, why not amputate it and create a prosthetic leg? Just things I'm wondering.
To him, its probably no different than being wheel chair bound. He'd never be able to move the same again

You certainly have a point. I'm going to say that either Gai was too proud to accept surgery/a replacement/or wanted to keep his leg the way it was for sentimental reasons (a memento of his fight with a God, if you like) or Kishi wanted to reinforce the point that his youth/glory days are now over and it's Lee's turn to take the mantle of gate-using moleman of the leaf.

I think there are many physical, emotional, and official reasons for Gai not being an active ninja anymore.

Let us first consider the physical. Gai opened all of the gates and launched an attack that had great backlash against his body. This can be compared to Lee opening 5 of the gates and using the ura renge. Lee had spinal damage, nerve damage, and muscular damage, so we can assume that Gai had at least all of this, and definitely more. His body was also on the verge of all turning into ash. Naruto was able to stop this reaction from affecting his body as a whole, but we can only imagine the internal damage that he must have received. It can be assumed that it was more than Tsunade would be, in good conscience, willing to operate on (Lee's lesser injuries had an incredibly low survival rate, so imagine Gai's.) Missing most of your toes also does not require a full leg cast like that. We must remember that he also broke just about every bone in that leg, in ways that could not possibly heal correctly. Physically, Gai's ninja career was over.

Next, emotionally. Gai fully committed himself to his final attacking being just that: his final attack. He did not intend on surviving the technique, and in a sense passed on his will to Lee and the future young leaves of Konoha. His father, Dai, also died after passing on the technique and conditions to Gai, and Gai's most defining trait is the strictness of his own rules. In his mind, his life was forfeit the moment he opened the gates, and surviving and continuing to actively fight after that would be breaking his own rule of throwing his life away for the next generation. He has entrusted everything to Lee, so coming back and effectively taking back all that he entrusted is not in his character.

Lastly, the official reason for Gai not being active is that, as a Jounin, he does not NEED to be active. Historically (or rather, allegedly) Jounin were the ones that handed down the orders from the top of the ninja chain, with Chuunin being the squad leaders of genin, the genin being the ones who actually did grunt/wet works. Gai is still in the highest ranks of the Konoha military system, despite not being a front line fighter. This was demonstrated in the movie when he was in uniform beside Kakashi directing the taijutsu assault against the incoming meteor. Gai can lead Chuunin and genin from his (wheel)chair, like upper ranking military do.

Well said, I get the emotional part of down pact, but the physical is still a blank. Trying to apply logic to it won't clear it up, logically Tsunade shouldn't or couldn't have healed herself the way she was, technically she should have been dead. Who knows, maybe her working on Lee - may have unlocked hidden truths to a new regenerative technique that she used on herself, that can heal nerves, bones, muscles and all. Mind frame wise, we could apply that to Obito who was prepared to die once he threw Kakashi away before being crushed, half his body was crushed, along with with half his skull, just as much never ending damage.

But, maybe Gates not only intensifies attacks, but damage also, that would be a clear bell.

I believe that the gates do just that. They increasingly damage the body as they increasingly raise your strength, and this is why it is a forbidden technique (along with the fact that you are supposed to turn into ash after it is complete.)

Tsunade cannot be compared to Gai for the fact that medical ninjutsu is NOT what she is using to heal herself from grievous wounds. Rather, she is using a sealed, forbidden regeneration technique, something which cannot be applied to others. Perhaps if she was able to store the necessary amount of chakra in a remote seal on Gai's body for years and then activate she would be able to heal him, but by then his wounds would be treated by his body as its default state, and no regeneration would happen anyway.
