Well at th beginning of the mange Sasuke said that not everyone unlocks sharingan so Tobirama must have had on mind those who awakened. We saw that Sasuke awakened it in a moment of crisis too (Uchiha massacre and vs Haku) so seems legit.

Well we saw Sasuke and Obito when they unlocked their basic sharingan and i cant remember they lost someone... They were just in trouble.
Tobirama was clearly speaking about Uchihas who lose someone they love. So i thought he was talking about ms.
