It is revealed that Juugo's original curse seal is the Dragon Sage mode, but since he lacked the chakra to balance the natural chakra perfectly (and his ability of constantly absorbing the natural energy only made it worse) he went berserk.

This also explains why his physical form also radically changed.

the sage mode in all cases can only be beformed if the user has trained with the original sage weather its (toad, or snake) and gather and is able to master nature chakra. the curse seal has nothing to do with the sage mode kabuto was using juugo's clans ablitities to enhance his ablitities more in terms of mastering the sage mode

That's interesting orochimaru only ripped it off and instead of the nature charka the user of the curse seal is actually using orochimarus charka
