So Madara has susano'o but ...

No indication that Madara's Infinite Tsukuyomi is the same jutsu as Itachi's, because Madara has only been shown casting his without a Mangekyou (he used the random spare Sharingan after giving his Rinnegan to Nagato). Tsukuyomi is a moon deity, Tsuki no Me is the Moon's Eye Plan... lots of moon stuff with the Uchiha. NO indication that Madara truly has a Mangekyou-specific Genjutsu called Tsukuyomi / Tsukiomi / etc. @

In the end, any complete set of Mangekyou could probably awaken a Susano'o, regardless of which two Mangekyou abilities are mastered. HOPEFULLY we see Madara's abilities against Hashirama.

I think all MS Sharingan users have the standard Amateratsu, Tsukuyomi, and Susanoo. I think they just have a preference or affinity to one of the 3 abilities mentioned above. For instance:

Itachi-As a master of genjutsu he specialized in Tsukuyomi

Madara- Havin perfect Susanoo leads us to believe that his preferred ability is Susanoo.

Sasuke- though we haven't seen much in the way of his EMS abilites, we know he can wrap amateratsu around Susanoo as wellas have Susanoo use Amateratsu in the palm of it's hand as he demostrated against Zetzu. So I would assume Sasuke prefers Amateratsu

no he doesn't. all you have to do is awaken both MS eyes (or master them) in order to bring out the 3rd power. Sasuke says then during is fight with the Raikage. I could go more into detail about it but this is the basic answer.
