This pic was shown in anime during the fight between itachi and sasuke.....And itachi said that it is the first mangekyo sharingan of madara.......So was this the real pic or they just add it.......But man if it is true then it's hell lot cooler then any other i have ever seen.....Just look at the devil figure....Damn it's cool....

That is Madara's EMS. I believe that image you posted is meant to signify the fact that Madara got another set of eyes, ACTIVATING the EMS.
